Paying too much for web hosting?

I am needing a Userform or other device that will allow for speedy input of hand
written medical notes and place the data into specific parts of sentences and
charts. Some of the data should be displayed in other portions of the report as
graphs comparing previous findings to the current findings. I am not aware of
all the available options so creativity and suggestions are appreciated. I have
researched UserForms and that appears to be a workable format. However, I am not
tied to anything at the moment. The data can be contained in Excel 2003 or
Access, or anything else that I have or is easily attained.
I have an Example of a UserForm that I created to give you an Idea of what I am
trying to do. This is very crude and my first ever attempt at a Userform, so
don't feel constrained by the format. Also, I was given an example of an invoice
Userform and permission to use the code, I really don't know what to do with it.
I have attempted to put some of it in my template, but it did not work, so just
disregard. I have included the Invoice Example that I was given also, in case it
helps.
The main concern with this project is speed. Speed of data entry/report
generation and speed of getting the thing created and working.
I am creating two types of reports, SOAP notes which are a daily record of the
patient encounter, and Examination Narratives, a more formal report with text
tables, charts, and graphs that describe a patients response to treatment, and
other detains over a given period.
I have attached an example of what I want the finished SOAP's to look like when
completed in groups. If they are created individually, then there will be only
one note created, But I have to have the ability to compile the reports for
several successive patient encounters for a given patient. Also, each successive
daily record should appear different from the previous. I am simply trying to
avoid a "computer generated" appearance. Simply recording the text within the
paragraphs is sufficient, and if feasible the random use of synonyms for certain
phrases would be ideal. Each SOAP note is a record for one day. Each note will
contain between 4 and 6 sections. a Subjective, Objective Assessment, Plan,
Goal, and Today's Treatment. All daily notes will have SOAP, and the Goals , and
treatment sections will appear only if there is data available for insertion.
i.e., if no data in Today's Treatment, then no section is produced in the word
document. Each word document will contain multiple instances of daily notes. For
instance I may include 12 Daily SOAP Notes, into one word document based on a
range of individual patient encounter dates.
Subjective
The subjective section describes the patient's current complaints along with a
rating of those complaints on a scale of either 1-10, or 1-100, depending on the
intake form that is being used for that date of service. This data should be
input from list, clicks, etc. And should default with the most recently entered
info so only the data that require changing would change. Additional information
should also be allowed for direct entry and the end of the above sentences.
Answers derived from a questionnaire should also be easily input into this
section. Items such as "The patient reports that he has difficulty standing for
more than 20 min without pain"... etc....I will provide a list of a few of the
statements via email, an I will bee adding more myself.
The rest of the sections I can elaborate on later, and the examples are pretty
self explanatory.
The examination is not nearly as involved as the SOAP. There is no concern with
the issue of syntax or visual randomization. Appropriate fields should be
populated with information entered, and the Graphs should be created based on
the appropriate data sets in the current exam and provide comparison to previous
EXAMS. The tables will be populated with input from the user and saved in the
data source.
I am providing links to an EXAMPLE SOAP, an EXAMPLE EXAM, and basic EXAMPLE
USERFORM that was suggested as a guide and is linked with permission of the
author for use and linking, an EXAMPLE SET of notes, I am working on adding
comments to expound on what I am looking for, but it is just partially finished
at the moment.
All data entry Fields MUST be populated with the data that was entered on the
last visit, so that all items do not have to be re-entered. If the answer is the
same it can be left in and used as the value for the new record.
I will need the functionality to modify all the lists and text, and add
additional items to the lists/option boxes. I will provide you with the basic
list items and standardized text lists. However, the ability to change, add, and
modify is essential. A brief instruction sheet needs to be created giving basic
details on how accomplish the editing.
Bid acceptance while not be limited to price alone. Quality, creative
suggestions, and turnaround time will play a major role in the selection
process. Also, I am open to suggestions and I am not limited to userforms. If
you have any other ideas, feel free to suggest them.
I am sure I left out a lot of info, so just ask if you need anything more.
I will attach the template of my example and will make links to other examples
