Description:	I need a simple form created that will gather information. The form needs to use PHP and validate the information entered. The following information needs to be emailed to an email address: Name City Phone Email Reason (This will contain two list items to chose from: Foreclosure or Other) After the information is submitted, the user will be directed to one of two possible pages. This will be determined by the last list item on the form. If they select "Foreclosure", they will be redirected to "reports.html If they select "Other", they will be redirected to an undertermined HTML page. Nothing too fancy, just a basic form using PHP to process the data and send to an email address and redirect to one of two pages.
