Description:	I have an application in word doc with approx 150 fields, that i would like converted to a form and have paypal integration, meaning after completing the application it would take them to pay with paypal...I would like to be notified by email of any submission as well as i would like for the applicant to receive a confirmation email, and i would like the fields to be stored in a mysql database as well as go to an email address.
