The program will be built in software on a small USB Flash Drive makes it accessible in seconds on any windows based computer, and allows it to be carried on key chain, purse or wallet making it fully portable. The software will be on a usb key that is a self-contained device that works independently without the need to install any software onto any computer. The program must have the ability to add pictures per the various forms that will be included when printed the pictures must also be printed. The program must have sections for "miscellaneous" uploads, The software program has to store information digitally, on an easy to read form with an emergency information area highlighted to get attention. Also the user must be able to scan his or signature and save it in a document that requires a signature all fillable forms must have a signature section. Some of the forms that will be on the program will include but not limited to, medical release forms , wills, Financial information, legal forms, DNR do not resuscitate forms, (we have some of the forms that we will provide and will need to be placed on the program) Also must be able to scan other documents such as a medical prescription, fingerprints, medical images such as X-ray, MRIs dentals, birth certificates ,ect, video clips, various images files and more. Other information on the program will be the graph lab value medical history, laboratory values, medications, vitamins and supplements,immunizations, family history, medical images, and more doctors and insurance information, , medical records, blood glucose levels, weight loss, cholesterol levels, and other data, create personal reports, track fitness goals and stores, MRI's and EKG's, financial information and other electronic downloadable forms such as poster. The program must also have the ability for a someone to prints posters/flyers at the click of a button and documents and save them in their specific section. Again electronic copies of vital documents such as X-rays, scanned medical images, advance directives or medical test results can be attached to the KEY. Store documents: living wills, medical power of attorney and consent to treat forms, Store photos, fingerprints, emergency contacts, medications, medical images, surgeries, hospitalizations, family history the software will have a public access records that can be viewed by anyone to launch critical information in emergencies with basic life-saving information such a medical profile and the phone numbers The public information will appears each time the program is ru,n with full-color image, details about the individual's medical condition, special medications, emergency contacts, and other information useful to first-responders. Also there will be an option to password protect specific items in program. The owner will determine what information will be viewed by the public and what information will be restricted. software program has to be both password protected and fully data encrypted. In the unlikely event that a user loses his or her USBstick and someone else tries to read the information, without the password, the data is viewed as un-readable due to its sophisticated data encryption technology. This program will be installed in a portable flash disk and can be downloaded also on a PC and or a floppy disk and must be universally accessible via all computers and or web portals. The software must be user friendly, plug and play? and the information form has to be a simple fill in the blanks format and or check the box format. Must be able to update and save information for multiple individuals each having his or her own data. We have some forms that we will provide samples and will possibly have to be recreated. Again must be able use virtually from any PC or Macintosh, Again must be able use virtually from any PC, MAC computer with windows 98 or higher 64MB of ram or higher. All the information entered onto the usbKEY can be backed up to a location on the owners hard drive in case the key becomes lost or damaged.
