Netfirms Web Hosting for Small Business

We need a form written in PHP to put on a website. This form will have to be
extremely user friendly and easy to edit.
The form will include several fields, that have to be completely editable (font,
size, colour, bold, italics, underline)
Each field will be put onto a certain sized “ad” which will have to preview in
real-time using JavaScript/DHTML.
The form must measure 710 wide.
This code must work on Firefox and IE. I will see the preview of the working
form completed on your website before we pay.
Please look at this picture for an example of what we want.

This must be finished in 5 working days after commencement.
