Description:	Hi - I need a poll created for my site. We are picking a new logo and wish to allow our users to help in that process. Basically, we are going to invite thousands of people to 'vote' on our new logo. So, I will also need you to create placeholders for up to 10 logos - I will provide them for you later today, but figure they should be on the top of the page, with a number identifying them. So I want the user to come to the page, choose their favorite 3 logos. Then I want the user to put in their email address if they wish to be notified when the voting is completed. Then they press submit (and i will create a 'thank you for voting page'). I don't care if people vote more than once. I need this done this weekend, I will pick someone asap.
