Description:	Hi, What I'm after is an online questionaire I can direct users to, which will be emailed to me with the answers once completed. I will need to collect the following information off my users. 1)Name 2)Address 3)Phone Number 4)Password 5)Other info1 6)Other info2 7)email address 8)Any other comments. I then need a paypal subscription button on the same page, that incorporates the information collected in 6)other info2, so for instance when clicked on - prompts the user to start a subscription at £1 p/m for other info or a buy-now button with the other info2 included as a product name. (hope that makes sense) Additionally I want to be able to direct users to the site with a reference number - for instance mydomain.com/questions.htm=12345.
