Description:	I would like to add some form fields to the following progress bar upload form. Uber uploader Demo: http://www.webdice.org/uber_uploader/ http://sourceforge.net/projects/uber-uploader Demo: http://www.webdice.org/uber_uploader/ The form fields are pretty standard, it asks for name, address, phone, email etc.. and obviously also asks for a file upload. (I'll give you a copy of the html form) After the form is submitted the file should be placed in a directory and an email should be sent to the admin with the form info and a link to the picture. It would be nice if the file was attached to the email as well. The user should also receive an email thanking them for their submission. The admin should also be able to define which files types can be received and max file size by editing a line of the code. You should be able to get this working using PHP and Perl/CGI on my server.
