I am interested in having a web-based survey system built for use by an independent market researcher. This is the process that I am envisioning: 1.The user will log in and create a questionnaire/survey. 2.He/She will then enter the list of email addresses that the survey is to be sent to (or select from previously defined mailing lists). 3.A link to the questionnaire/survey will then be send to the respondent 4.By clicking on the link the respondent will be taken to the survey, which they will fill out and submit. 5.The information collected will then be stored in a MySQL database 6.The user should then, with a click of a button be able to export this information to Excel and/or SPSS format so that he/she can analyze the data User = Web Based System Administrator Respondent = Survey Participant Features of this system: 1. Support for all popular question types - For each question added to a survey the user should be able to specify whether the question is an a.Open-ended questions b.Single selection question (‘choose one') c.Multiple selection question (‘choose many') d.Question that require the selection of a number from a specified range e.Rank order questions f.Multiple choice battery/matrix style questions 2. Unique respondent ID - Each respondent will have a unique ID number so that the user can match each set of responses to a particular respondent as well as keep track of which participants have taken the survey so that reminders or thank you emails can be sent to the respondents. 3. Support for Fact cards: The user should be able to add a ‘Fact card'. (A fact card prompts the respondent for answers. For example, if the question says “What is your favorite kind of meat? Tell us why.” The fact card will list fish, chicken, turkey etc. to remind the respondent of what all the different kinds of meat are. 4. Auto-reminder - The system will automatically send out reminders at particular intervals to respondents who have received the initial email but have not yet responded. The user should be able to set the time interval for each survey created. 5. Automatically generated thank-you letters - An automatically generated thank-you message will be sent after the respondent has completed the questionnaire. 6. The ability to limit the number of responses – The user should be able to set a limit to the number of responses to a particular survey that the system will accept before the survey is automatically closed. 7. The ability to limit responses to a specific timeframe - The user creating the survey will be able to specify the ‘life' of the survey i.e. the period throughout which the survey will be active. After the ‘end date' the survey will be deactivated and respondents will no longer be able to fill out the survey. 8. Response Validation 9. Unique Survey URL 10. User Defined Mailing List Database - The system will allow the user to create and save mailing lists. At the end of the survey creation process the admin should then be able to select from a list of already defined mailing lists. 11. Question Library – Allows the user to save groups of questions for re-use. 12. Username/Password security and support for multiple users - The system will allow for multiple users to be created with different privileges. For example, the ‘super user' will be able to carry out all tasks, but another type of user may be restricted from deleting entries, modifying information etc. 13. Single Response Validation – Each respondent will be allowed to fill out each survey only once (unless the user specifies otherwise). 14. The ability to export raw data to Excel/SPSS Format 15. Spell Check – The program will automatically check for spelling errors before the survey is sent out 16. Search and Replace – For quick editing of existing surveys 17. Survey Preview 18.
