Description:	Either create or modify existing scripts for uploading form info to a website. Can supply similar scripts, scripts found at HotScripts for slight modification, or write your own. Only code is needed, no design. See attached file for upload form inputs for these 2 forms. All of the form info can be saved in a txt file that is saved to a physical path (ie C:\Inetpub\data\ ).
