Description:	Hi, I need a PHP script so owners of pets who have passed on can create a memorial page for their pets. The users need to be able to upload some pictures, enter information about the pet, description and stories etc... all from one form. Once admin approves the submission a memorial page will be created using a template. The memorial page needs to have an html extension. It can use PHP as the .htaccess file allows this. There needs to be an admin section to set up the templates, review and apporve the submissions. The admin and user will be sent an email upon submission and the user will then be sent another email upon approval or denial. The form needs to capture the IP address of the submitter. I would also like a Tell a Friend script included in the script so they can tell friends about the site and also so they can tell friends about the web page of their pet once approved.
