- what i need done
1. When you check more than one box, it does not submit all the boxes you
checked
2. Formatting: please make 2 formatting changes
- line up the check boxes in 2 columns (see how they are off a little?). Same
allignment, just justify the right three checkboxes so that they are all in a
line.
- bring the input box lines up a little so the entire form doesn't go so far
down the page.
3. Make email and message required (and put a * to indicate that)

1. Format so it's like the previous...just make it clean and professional
2. Make the message area prepopulated like on this one:

edit it, add their own message).
3. When a customer enters multiple addresses, I would like to know who it was
sent to, but I DO NOT WANT each 'friend' to also see all of the messages (ie,
basically, the friends need to be bcc'd, but I would like to know on my end who
was bcc'd.) Does that make sense?
4. take out the box for FRIEND's NAME (as if they are typing in multiple emails,
that would be confusing). Everything should be required (so indicate that in the
description).

Will select someone once i have a few bids. Don't bid unless you are sure you
can do it. Must be completed within a few hours.

NO JAVASCRIPT PLEASE. CSS, or tables are ok if necessary as I think they are for
forms like this.

EDIT: please post a message saying you can work on this project
now, and can complete it. I need to know this can be done...
This isn't mean to be harsh, just that the first time I posted this the
programmer went MIA when I chose him. NO biggie, I just want to do this soon.
So please post to confirm your availability and confirm that you can complete
