The desired end result will be two interactive forms that collect data into a backend MySQL database for the client to download via query/report and use for administrative purposes. We are currently hosted on a Linux server with PHP v4.2.1 and MySQL loaded. The first form will be a registration form for an academic program, with fields for contact information, class selection, and acceptance of medical, liability, and photo releases. There is pricing based on which class is selected, and the form would contain the simple math required to compute totals and display that information. There will be fields for credit card information as well, but it will not be processed, only submitted to the database. The current form is approximately one page. The second form will be a general admission ticket sales form for an event. It will contain fields requiring contact information, event and section selection, credit card information, and other preferred fields. Please note that while the form is to collect credit card data, there is no present need for the application to process the transaction – simply collect the card information. The winning bidder’s responsibilities will be as follows: 1) Design both forms with PHP to our satisfaction for display to the end customer. 2) Build a MySQL database to accommodate all information from both forms; or two separate databases if necessary to separate the information. 3) After completion of the programming, develop any queries/provide basic training and related other actions necessary to query/report/retrieve the data from the database to the client’s workstations via the MySQL admin program. A contact person/liaison will be appointed by us to work with you and receive deployment, usage and troubleshooting assistance as required. Whatever needs to be done in these regards can be accomplished remotely and/or via phone/email or other communication. It is anticipated the selectee will be compensated for any out-of-pocket expenses in addition to agreed fees. Agreed fees will include basic development cost plus an agreed hourly rate for ongoing assistance, to be paid as utilized. 4) Completion of the project, including successful testing, will be necessary on a short timeline, with expected go-live date of 1 August 2006. We expect any bids to account for all responsibilities, and money will be held in escrow during the progress of the project. No funds will be released prior to successful completion. We will have additional similar projects for this client and others, so the winning bidder may well have the opportunity for more work with us after this project.
