It needs to ask for: "your name" [error if left blank] "your email address" [format to be tested for errors] "name of artist / band" [error if left blank] "you web site address" [http://] [error if not added to] "which of these areas are you from?" numbers 1 to 8 (8 tick boxes - more than one may be ticked) [error if no box ticked] "which musical style best suits the track you're sending?" (7 tick boxes): acoustic / indie / rock / dance / chillout / pop / classical [error if no box ticked] "how many months would you ideally like your track to be displayed" - dropdown (2 / 6 / 12) "please click here to (a) state that you are the artist or the artist's representative and (b) that you consent to the material being added to the player (tick box) [error if not ticked] Then browse for two attachements... - track [must be mp3 format - max size 12MB] - image [must be jpg or gif format - max size 100 K] .. and send!
