Description:	I need a paypal subscription order form created. The form will have the following fields: First name, Last name, Email and a drop down list box with product names and price which user can pick from . - Once the form is filled out the user should be sent to paypal and once payment is made the user should be sent to a thank you page and also sent a confirmation email with the detail of their order. - Header, footer and css file needed so that I can easily setup the form on my site. - Script should have a config file where I can enter the paypal email account, subscription period and add products which are displayed in the product drop down list.
