I think this is a pretty simple script. Please read all the way to the end so you understand the complete project and instructions and make your bid accordingly. No stock comments will be considered. Here is what I need. I need a PHP script that will make a certificate. This would basically be a fill in the blank type thing that would have a template that has basic info in it already and then I will fill in the blanks so that it would complete the certificate. The form fields would have the following form entires. Type Complete Name mailing address including street, postal code, city, state, country, etc. phone number gender age profession (drop down menu) Level (drop down menu) Classification Expertise START Date Completion Date Photo (would be an upladed graphic - jog, gif, etc) Awarder These fields would then print out on a certificate in the appropriate areas. (Note, not all fields will be placed on every certificate. It would depend on the type of certificate) The base certificate would already have standard info on it and the fields would just fill in where it is suppose to. The standard info would include 2 graphics in the bottom corners and a very light watermark graphic that would be in the backgound of the certificate, plus additional text info that would be a part of each cetificate. Once I have completed the entries on the script, it should do several things. 1) Save the info to a database 2) Edit entries 3) delete certificate 4) Ability to print certificate. - when printing certificate, it shoud request template name and then fill in the correct info based on the certificate template. - It should be able to print the certidficate two different ways. (1) Normal based on the template. (2) Just like the normal way, but with the words "Unofficial" that would print out diagonally across the complete certificate. I should also be able to dynamically show the certificate with the unofifical method on the internent. I guess basicallly, each certificate should be assigend a certificate number that could be automatically generated (prefeable) or I could assign a number manually and when that number is typed, it could show the certificate with the unoffical label on it and maybe a little distored and more miniture. The certificate when printed out should be able to be printed on a regular 8.5 by 11 size page. I also need you to design 2 templates for me. (but I should be able to easily add additional templates on my own) both templates should include a nice (fancy would be good) border like you see on a lot of certificates.
