php ver 4.3.10 MySQL ver 4.0.22-standard I have attached four files which outline what I am after. The user here creates a html form such as the sampleform.htm These are the possible form elements that will be used. The user then pastes the html code into the sampleform.htm and enters all necessary information as you will see. The code you will write will create a new form (in this case e1001.php) which when viewed will look exactly like the original form (in this case sampleform.htm) complete with default values. However all created forms (in this case e1001.php) will need the following code inserted: -php database connection strings as provided in the form -php code to load form with default data if no record is specified -php code to display specific record when provided as e1001.php?id=12 -php code to modify form tag to submit to $PHP_SELF -php code to add new records to database -php code to modify records being edited -php code to redirect on successful writing of database information The code that creates (in this case e1001.php) also needs to create the corresponding table in the database provided. As a final touch, the FormIndex, and FormSubmissionsListing pages need to be made to manage forms and access information submitted using the forms we've created.
