Description:	Ok, I need custom form processing - but more complicated than it sounds. I've got scanned copies of standard billing forms. I need to be able to populate relevant fields from MySql (all fields to be called by initial entry of id number). I need to be able to print the data to the actual form (about sixy fields on one page - all need to be perfectly aligned). I need to be able to export the data as pipe delimited. I need to be able to manually enter information that is not already present in the system (stuff not populate by MySql). My initial feeling is a PDF that uses PHP to populate fields and also allows for keystroke entry. That form then gets printed AND exported AND added to MySql. And I need the whole thing commented.
