Software must be PHP with database MySQL. Hopefully you have done something similar you can use as a starting base. PLEASE LIST ANY SIMILAR URL'S WITH YOUR BID. I will probably be integrating with PHP-FUSION or what ever other site I choose, but that shouldn't effect this project. So you are not designing the whole website, just the functions described here. I want to create a directory of churches, synagogues, or orther various types of organizations. A leader from any organization should be able to sign-up as a new user. System should email verification code before complete survey can be filled out. Leader then fills out contact info and survey questions that describe the organization. These columns always present: 1) Organization Name 2) Address1 3) Address2 4) City 5) State 6) Zip 7) Country 8) Comments (up to 500 characters) 9) Website 10) Email 11) Metroplex area (for example Dallas/Ft. Worth, even if your city above is Arlington). 12) Contact First Name 13) Contact Last Name 14) Phone 15) Fax There should also be a userid, password, email with typical register new user, "lost my password" functions. Additional columns should be database driven, for example, a table called UserDefColumns might have the following values (Please use these values for any demos and testing. I will supply full questionaire for testing after bid is won. You must test and demo with all questions I send - approximately 20). The Score will be used in future reporting (should be a number from 1 to 100). Example Questions: Sidur/Liturgy: A. Traditional Sidur is used B. Messianic Sidur is used C. Liturgy is on handouts or on overhead D. Very little liturgy is used How many hours of classes are available per week outside the normal services? A. 5 or more hours B. 2-4 hours C. 1-2 hours D. 0 There must be admin screens to allow for data entry (add/upd/delete) of all the above tables. Another function is that I should be able to hire someone to enter the basic info. There should be an email letter (stored in database) that can be sent organizations stating that there basic info has been added, and they are requested to complete the more detailed survey. There should be a flag in database indicating that data has been entered internally, and a bulk email function to invite organizations to participate in survey, along with unsubscribe option. I have one or two specific sites in mind for this, but would like to have the software generic enough to resell (This is a work for hire, I maintain all rights to resell). I prefer to work interactively, where as you develop, you send me what your are working, I critique or approve. If desired, I can do partial payments as we go. If you have any other ideas for anything I should have thought of, please let me know as we work on the project. If you can recommend an off the shelf product that does what I need, and I select that product instead of selected a freelancer, I will pay you a $25 finder fee. All (1) data entry, (2) admin screens, and (3) user reports should be able to be customized by changing CSS file(s). Could use one CSS for each. Reporting/Searching for website visitors shall include: 1) Full listing 2) Search by country (pull-down list) 3) Search by State/Province (pull-down list) (US/Canada Only) 4) Search by Metroplex (pull-down list) Optionally enter how many matches are to be shown per page (next page, prior page functionality required). The results for the above four reports should show basic info in a table format (with css). Organization Name City, State, Country WebSite (with hyperlink) Advanced mode: User should be shown list of all data, and can select which data to be shown (and also what sequence). User may select results of survey questions to be displayed in a column. 5) Search by any three survey questions: User should be able to select one, two, or three questions and desired matching answers. Pull down boxes should allow multiple selections (for answers). For example, using questions above: Sidur/Liturgy: A or B Hours of Classes: A or B or C Kipa Required: True 6) Side by side view - user should be able to select either two or three organizations (probably a checkbox from lists 1-3 above) and see all the survey results side by side, i.e. each organization goes in a different column (report would be either two or three columns wide).
