Description:	I have a fairly complicated PROFILING / Registration form. I have created a couple of pages using CoffeeCup's Form creation program ( see http://www.coffeecup.com/form-builder/ ...for a free trial version) and have included them / attachment. Also I have an example MS-Access 2000 .mdb file to illustrate my needs in that form/look. This needs to be a PHP/MySQL online form however. I figure it needs to be four or five tab pages. I also have created the MySql files as well using PHPMagic (which I can send you to further develop...as I only know how to do the basics). This form needs to save the entries in the MySQL database and registrants (members that sign up for trial or paid memberships) need to be able to come back to it from time to time to update their PROfile information.
