Project Overview and General Guidelines. ON LINE PATIENT REGISTRATION
Description of entire project.
There will be approximately “2- 3” input pages / screen dependent on your
layout. All of these input pages will be for ONE record. Each page will need
to have a submit button for updating database. The input page data will be
“temporarily held” on client computer. When all pages are filled out the data
will be sent (secure transmission) to server database for retention.
The structure of XML and Xforms must be in W3C standards.
The project includes both the development of “Xform” input pages, viewer, and
Linux database. The pages will look like MS Word docs I send.
The data must be retrievable and able to be used to update other PDF forms (with
same tag identifiers) and print at client location. There maybe up to 4 to 5
PDF forms to be printed. There must be a feature developed in this project to
incorporate the option for printing of these other forms. (the production work
of the PDF’s is outside the scope of this project … but will need to be
completed later)
Successful bidder must be very experienced in W3C standards for XML and Xforms.
All source code must be submitted and pass tests of it being “well” formed. See
w3schools.com pages if there are questions on “testing” form documentation.
Successful bidder must be able to “demo” their work before final payments are
made. Money will be deposited and escrowed into an account.

why there is a need for something else.
