Description:	I need the following survey of http://www.tatadmin.com/tat_member_survey.htm collected and stored in mysql database. I need an admin area that will show each individual survey, date and time collected and IP address to know if one computer might be taking the survey multiple times. I'll also need a total of each selected item per line number for all surveys taken as well as the ability to delete surveys subtracting from the total if an address is taking it multiple times.
