The current javascript only checks for witness 1. Please fix. 2) Validate at field level (onChange?) that all job dates are in proper format of mm/dd/yyyy 3) Validate at field level that all the dates and times are at least 48 hrs in the future from the date and time that is already captured from form user's computer clock 4) Fix current javascript for all (3) zip code fields to accept a blank field after onBlur is executed. 5) Autoformat all (3) zip code fields to put in the hyphen "-" when a zip code is typed in longer than 5 digits. Similar to the javascript code that auto formats the phone # field. The code should automatically put in a "-" after the fifth number if the sixth digit enterered isn't already a "-". Current javascript requiring Zip codes of 12345) or 12345-6789 format needs to still work when you are done. 6) Limit File Types that can be uploaded into the form to pdf, doc, wpd, jpg, tif, tiff, wps. Add an alert letting the form user know what is wrong if they try to upload a file other than ones listed. 7) Update perl file so it will add the information enterered into witness name fields to the subject line of the email i.e. (SchedulingAttorney - NumberofWitness0, NumberofWitness1, etc. 8) Make help fields work on mouseOver and mouseOut from the ? icon and get rid of the X (close icon). Said in another way. The help field should be reveiled when the user simply moves the cursor over the ? icon. The help field should close automatically when the cursor if moved off the ? icon. Please only post bid if you know you have ALL the skills to perform the above requests. Please also note that one of the above needs requires Perl knowledge.
