I need a paypal order form similiar to what you see here at:
http://www.trafficdeals.com/country.php
It will be similiar but with extra fields added for extra targeting with the
price being reflected. Not even sure it can be done, but if you have another way
with something similiar that can get this done, great.
It will have 1 main field to pick from.
It will have a http:// field to add url
(UPGRADE FEATURES)
It will have a option to pick language.
It will have a option to pick Country
It will have a option to pick keyword
It will have a option to pick extra keywords

Set this on a page so I can take a look at it.
It must be easy to copy/paste into a html, or .php page.
After completed, must provide support for 1 week for any changes that are
needed.
