this project. We will own all source and rights to this project when completed. Programmer can use project in portfolio as a refference when completed. Finished product will be unbranded by developer. I have a working system but is comprised of several modules using excel and access. I need this converted to a single, easy to use interface. A single access interface is acceptable. Patient data entries to include: Name last , first DOB: MM/DD/YY Chart ID: XXXX1234 StaffID Addl data: 2 basic types of serum A and B types We need 12 place holders for each type (24 total) Each place holder will be 1 of 2 amounts .50/.050 or .25/.025 and have a name The amount will be subtracted from a fixed amount 5 or .5 The total of the .50/.25 will be subtracted from the 5.00 amount The total of the .050/0.025 will be subtracted from the .50 amount. The difference will need posted to an amount labeled "Diluent" This will post to a "Mix" sheet with the selected place holders, amounts, and patient info There are 4 concentrations used 1:50/1:500/1:5000/1:50000 , this applies to the mixed finished product. It will also determine total amount of serum. The 1:50 will use the .50/.25 amounts and the other 3 use the .05/.025 amounts. This also determines expiration date of serum: 1 year/1 year/9 months/4 months respectively. This will all display on a single page to allow staff to mix the serum and adjust amounts according to the concentration. (this should adjust with the selection of the concentration) Patient name: DOB: Vial Type (A or B) Concentration, Expiration Date: Date: Staff ID need to output to file for DYMO 330 Printer (SDK avail from DYMO) Access query can extract needed data also) This system must also print an "Injection" record sheet. It will contain specific text as to special instructions per patient as well as dosing amount, reactions to injection. (see attached file for examples of current setup, the final page of workbook contains the exported data that goes to access for label query. Missing are the links for our billing sheet, this is simply Patient name, DOB, number of vials. This project will require significant interaction with us until completed. If you can not respond in a timely manner do not bother with this project. Ideally, the completed project should be simple to use for anyone with minimal instruction. The shot record must be easy to edit and entries for injections must not overwrite prior entries.. All entrys to shot record must be protected after save.
