Description:	This is a realtively simple problem. I need a basic web form that allows a user to check a "Enter another" box to loop a certain part of the form. (For instance, as many transactions as they want). That data is then written to a CSV file. I tried playing around with PHP and javascript, but what would take me a few hours would take someone here a few minutes. Writing this in php, Java, or javascript would be fine. Just need a basic web form of which a section can be 'looped', and all the data written to a file. I have a server and can give you logins etc. The catch is that this needs to be done ASAP.
