Description:

We have a word document that people print, answer the questions and post back to
us.
We want to put this online.
So Users login with a username and password
Answer the questions online
Can save and come back later to finish off the questions
Admins can login and view the answers of the Users
Admins can have access to specific Users and there questions
The word doc to be put online is attached
The site will be done in ASP with an access database or MSSQL database, which
ever you prefer.
The Word doc has about 4 questions where pie/line graphs where to be filled in,
but to keep things simple you can change this to a rating out of 10 types of
question rather than coloring in a pie graph.
Please mention how you would deal with the pie question in your PMB so I know
you have read this job correctly
Also the questions are divided into 4 parts, the user should be able to go to
one of the parts, i.e. doesn't have to do them in order of part 1, part 2, part
3, part4.
Payment will be through escrow so you know you will get paid.

dont want anything fance with the pie chart as to keep the cost down, so you can
remove the pir chart questions compleatly and just have a questions with a
rations out of 10 to replace it.
